IBMS application form: new members, rejoining and upgrading membership

You can download application forms below or request forms and membership packs by emailing mail@ibms.org with your full postal address.

Please note:

this application form is for all new, re-joining members and those who are wishing to upgrade their membership for all classes
that to comply with laws concerning the security of holding credit card information the application form is in three parts. You must complete and return the applicant details and payment details. However, part 3 - equality and monitoring, is voluntary but its return will affect the monitoring of the organisation’s services to make sure these are fair and provide broad opportunity.

Please note that all applicants for Licentiate membership whether new or upgrading must endeavour to complete the proposal section of the application form. If you have difficulty in meeting this requirement, please contact mc@ibms.org in the first instance.
