This spreadsheet is to be completed along with the IBMS Laboratory Training Approval Declaration as part of an application for IBMS laboratory training approval. Please indicate which levels and disciplines of training approval are being applied for within the 'Overview' tab, and complete the corresponding tabs with the training programme information details (i.e. laboratory sections, rotation schedule, assessment methods, etc.)
